ADR (Adverse Drug Reaction) Reporting Form For Pharmacists And Healthcare Professionals

Pharmacy council Of India (PCI) has sent communication regarding  ADR reporting to all pharmacy institutions and hospital pharmacists in the country, in response to letter from Indian Pharmacopoeia Commission.
Adverse drug reaction is any unwanted, unexpected or dangerous reaction to a drug. ADR reporting by pharmacists, and health care professionals in general, is required to monitor the unwanted effects of drugs. Based on ADR reporting the rational use of drugs is promoted.
Under it’s Pharmacovigilance Programme Of India (PvPI) the Indian Pharmacopoeia Commission has communicated standard ” Suspected Adverse Drug Reactions Reporting Form”. All health care professionals Clinicians, Dentist, pharmacist, Nurses etc can voluntary report  Adverse Drug Reactions.

After filling the form it can be scanned and mailed to pvpi@ipcindia.net or call the toll free helpline number 1800180324, Monday to Friday 9:00 AM to 5:30 PM. App named “ADR PvPI” can also be used.
